• If you are an active student, you can log into Brown Student Health Services Patient Portal and fill out a Medical Records Release under the forms section.
• Nonactive or graduated students should print, complete and return a Student Health Records Request/Release Authorization.
  • Student records are permanently deleted 7 years after graduation or separation from the University.
• Mail to Brown Student Health Services, Box 1928, 69 Brown Street, Providence, RI 02912 or fax to 401-863-7953 to the attention of Christine Farland or email to christine_farland@health.brown.edu.

Blanket Release Policy Statement

• If you are an active student, you can log into Brown Student Health Services Patient Portal and print your immunization record. After logging in, select Immunization, View History and click on the printer icon.

• If you are not an active student, please email nursing@brown.edu or call 401-863-3953 to obtain a copy. Please include your full name and date of birth or banner ID in the email. We can mail, fax or email your immunization record to you.
